State of Connecticut Oxford members have access to the Oxford Freedom network, one of the largest networks in the Connecticut, New York* and New Jersey tri-state area, with more than 110,805 physicians and other health care professionals, and more than 200 hospitals.
Our State of Connecticut members also have seamless access to 937,524 doctors and 5,888 hospitals in our United Healthcare Choice Plus national network.
*The following counties in New York are considered within the Oxford Freedom network area: New York, Bronx, Dutchess, Kings, Nassau, Orange, Putnam, Queens, Richmond, Rockland, Suffolk, Sullivan, Ulster and Westchester. All other counties in New York would be considered out of the Oxford Freedom area, so members would utilize the UnitedHealthcare Choice Plus network.

When traveling outside of the tri-state area of Connecticut, New York and New Jersey, search for physicians, hospitals, laboratories or other health care providers within our national UnitedHealthcare Choice Plus network by name, location and/or specialty, or search by condition or procedure.
Please note the State of Connecticut has a Tiered Copay structure for Primary Care Physicians (PCP) and Specialists in the below listed specialties for services performed in the state of Connecticut only.

Visits to "Preferred"/Tier 1 PCPs and Specialists (those with 2 full blue hearts) will have a $0 copay; visits to non-"Preferred" (less than 2 full blue hearts) specialists not in one of the above specialties, or visits to Participating providers outside of the state of Connecticut will have a $15 copayment.
*Note: Non-HEP members utilizing a Tier 1 provider will be required to meet their $350 In-Network deductible prior to services being covered at $0 copay.

Site of Service Lab and Radiology in Connecticut
The State of Connecticut has a Tiered “Site of Service” benefit for Outpatient lab and radiology services. For lab and radiology services, members seeking services at a “Preferred Site of Service” provider will have a $0 copay; members who have services rendered at a non-"Preferred Site of Service" provider will have to pay, out of pocket, 20% of the cost. Failure to utilize a “Preferred Site of Service” provider could cost you hundreds of dollars in out of pocket costs. Please consult the list below to find a Preferred Site of Service provider. Note the list will be updated from time to time, so members should always consult the list prior to receiving lab or radiology services to avoid out of pocket costs.
You pay the standard copay for these services when they are performed during the course of an office visit at a physician or specialist’s office (for a Preferred Provider office visit the standard copay is $0 and for all others it’s $15). Imaging and lab work performed at your physician’s office is covered under Site of Service at no cost to you.
*Note: For Point of Service (POS) members with an Out-of-Network benefit who use an Out-of-Network provider for their lab and/or radiology services, they will be subject to their Out-of-Network deductible and 40% Coinsurance.
*Currently, this applies to providers in the State of Connecticut and Westchester County, NY. It will be expanded to all counties bordering the state of Connecticut in New York, Massachusetts and Rhode Island soon.
