Please note: Effective October 1, 2020, the State of Connecticut transferred administration of its medical benefit plan to Anthem. If you have questions about your benefits as of October 1, 2020 or for medical services rendered after October 1, 2020, please contact Anthem at 800-922-2232, from 8:00am to 5:00pm ET. If you have questions about benefits or services rendered on or before September 30, 2020, please contact Oxford Customer Service at 800-385-9055, from 8:00 a.m. to 6:00 p.m. ET, Monday through Friday.
If you used a Non-Participating provider for services prior to 10/1/2020, it is your responsibility to ensure that the claim is submitted to Oxford for processing prior to March 31, 2021. Please note that claims received after this date cannot be processed for payment by Oxford.
